Accidental overexposure to radiation from Co 60 source

TAEA was informed on Friday (8 January 1999) that an overexposure to radiation from Co60 source occurred (approximately 88 Ci) which was broken into pieces by scrap dealers. The capsule of the source was not ruptured. The experts of TAEA have found the source and transported it safely to the waste treatment unit of Cekmece Nuclear Research Center. Now the accident area is under control.

Loss of radioactive Co60 source of 700 Ci

Loss of approx. 26 TBq (700 Ci) of a shielded or unshielded Co60 source. Details are as follows:
1. The TAEA became aware of the event on 8 January 1999 when it was informed that a radiological accident had occurred in Ikitelli, Istanbul.
2. The ministry of health, police departments, the public and all relevant authorities have been informed by TAEA about the accident;
3. The inquiries and search of the source are still in progress by the experts of the TAEA;

Worker irradiation from a Co-60 source after a gamma radiography

On the evening of 29 September 1997 two workers from a subcontractor were asked by the contracting company to perform a non-destructive test with a Co-60 source of 1221 GBq (33Ci). The irradiation was expected to last for 7 hours. Owing to a misconnection between the remote control cable and the source holder (due to a human error in following the written procedure), the source was left in the exposure head at the end of the irradiation. In addition, the two workers did not switch on their radiation monitors.

Discovery of a Cs-137 radiation source on a scrap yard

On May 15, 1997 increased radiation levels have been discovered by routine control measurements on a scrap yard of a metal foundry at Hettstedt, Sachsen-Anhalt. A radiation source containing about 200 GBq Cs-137 has been found. The cylindrical source remained undamaged. Surface radiation dose rate is approx. 2.5 Sv per hour. No cesium contamination was detected. The source has been fabricated by CIS BIO International, France and was part of a scrap loading transported by a Dutch transport company from Hamburg.

Radioactive contamination of the fertilizer due to Co-60 source failure

The Sealing cladd of the radioactive source Co-60 used in the chemical reactor factory for the level measurement was detected as failed. Corrective actions initiated identified that the cladd was not sealed due to corrosion effects. As a result the plant was stopped and the operating licence withdrawn for the Co-60 radioactive source up to the clarification of the causes, implementation of the actions considered to be taken and evaluation of the consequences.

PERSONNEL IRRADIATION IN A THERMAL POWER PLANT DURING MAINTENANCE WORKS

During maintenance works at the Borzesti TPP non destructive examination had to be done for some pipes. The examination failed due causes unknown at the time. Further investigations revealed that the pipe NDE failed due to undiscovered initial failure of the Iridium 192 source supporting stem break, followed subsequently by NDE personnel failure to observe the specific procedures for the source manipulation. The source had an activity of 622 Gbq and 6 workers got doses of max. 75 mSv/person. A press release was issued on July 6, 1995, immediately after the irradation was confirmed.
