Loss of radioactive source (Cs-137, Bac Kan, 2015)

The lost radioactive source is Cs-137 (sealed source, code number: CS-137DT, made in China) with an activity of 4,5 mCi in July, 21st 2010. The source was installed in the level gauge, used to control the clinker automatic disposal system in the cement manufacturing factory of Bắc Kạn DATC joint stock company (DATC company), at Suối Viền, Xuất Hóa ward, Bắc Kạn city. At 8.30 am in 15/12/2015, legal representative of DATC company informed that the radioactive source had been stolen via telephone.

Loss of radioactive source (Co-60, Pomina, 2015)

The loss of radioactive source originated from liquid steel spill accident during steel billet manufacturing process of the factory at the beginning of September, 2014. In the accident, lead layers for radioactive source protection were damaged. The radioactive source was Co-60, its activity at this time was 2,33 mCi. After the damage had been detected, the technical team had removed the box for maintenance. The technical team proposed that the source should be isolated because its safety was not ensured.

Loss of radioactive source (Ir-192, APAVE, 2014)

At 12 am of September 12th, 2014, in Hồ Chí Minh City, staff from APAVE Asia Pacific limited company discovered that an equipment containing a sealed Ir-192 radioactive source with the activity of 20,5 Ci had been lost). After unsuccessful attempts to retrieve the source, the Company had reported to competence agencies on the emergency. By the evening of 18/09/2016, competence agencies has found the stolen radioactive source (remained intact in the projector) after receiving information from a man living in a house nearby.

Loss of radioactive source (Cs-137, Song Da, 2006)

On July 28th 2006, the factory temporary stopped production for equipment maintenance. On 30/07/2006, the radioactive source box is removed from the manufacturing line and put in a inox container with a layer of lead lining before putting on a cement pedestal for clinker disposal. The Cs-137 source's activity is 10 mCi. This was a sealed radioactive source, put in an protective lead box. The box's dimension was 100x100x120mm. In 8/8/2016, the company detected that radioactive source was loss. Despite best effort from relevant agencies, the source has not been found.

Loss and dispersion of radioactive source (Eu-152, 2006)

On May 29th, 2006, the Eu-152 radioactive source with an activity of 25 mCi at 10/1995 was lost when being kept at a temporary storage facility. At this moment, the factory was under repairment. At 10h30 of May 31st, 2006, the police office informed that the the radioactive might be kept at No 628, Bạch Đằng street, Hai Bà Trưng district, Hà Nội (an garbage collection area), The radioactive source container had been broken, causing contamination to the house.

OVEREXPOSURE OF WORKER DUE TO THE MALFUNCTION OF THE INTERLOCK SYSTEM

On Nov. 17, 1992 a radiological incident occurred in the Vietnam National Center for Sciences, Hanoi. Due to the malfunction of the interlock system and bad co-ordination the operator started up the Microtron MT-17 while one of his co-workers was manipulating sample in the irradiation compartment and his hands were seriously irradiated by Bremsstrahlung radiation during two-four minutes. Twenty days after the incident the hands of the victim appeared to have been deeply burnt. The victim has been under medical treatment since then.
