Overexposure of two industrial radiographers

Two industrial radiographers who were involved in an oil refinery projects, overexposed by Gamma radiation of Ir-92, 35 Ci. On 23/09/2015, one of the radiographers dismantled the guide tube without noticing that the source/holder was detached and stocked in the guide tube due to not having survey meter with him. He put the guide tube in the car between the right front chair and back chair. During the night, one of the radiographers slept in the left front chair for about 6 hours while the other slept in the right front chair for about 4 hours in different times.

Stolen of a radioactive source

According to the report of Tabriz University Radiation Protection Officer (RPO), a Pu-Be Neutron source was stolen from storage location. National Radiation Protection Department (NRPD) immediately involved in the subject and informed responsible organizations in National and Provincial levels. The source was registered with the activity of 130 GBq (3.5 Ci).

Loss of Radiation Source

Refer to the event reported on 2009/09/07 on loss of a Neutron Source (Am-Be), after intensive investigation, the source was found and returned to the Atomic Energy Organization of Iran (AEOI) for respected QC and recertification.

Over-exposure of one worker during the industrial radiography activities

On 24 July 1996 an Ir-142 radioactive source used for industrial radiography and weld test was lost in the site of Gilan Combined Cycle Power Plant. Two hours later it was found by the safety authorities of the site and placed in its shield immediately. During the said time period a worker who had not observed safety measures, was exposed to a dose, exceeding annual dose limit (INES Level 2). He is now under full health care. The site safety authorities and the inspectors of AEOI took all necessary safety measures.
