Event during operation of Zaporizhzhya NPP Unit №3

On 28 of November 2014 at 19:24 the disconnection of the Unit №3 of Zaporizhzhya NPP from power grid occurred. According to the preliminary information the cause of the occurrence was the failure of electrical equipment in the turbine hall.
As of 3rd December 2014 the reactor facility remains in the “cold” shutdown.
According to the preliminary evaluation this situation is qualified according to INES scale – “0-deviation (No Safety Significance)”.

Ukrainian Nuclear Reactor Shut Down Over Malfunction

Kiev, 25 July: The third unit of the Rivne nuclear power plant was automatically disconnected from the Ukrainian power network and shut down on Thursday [24 July], the Emergency Situations Ministry said.

The ministry explained the shutdown with the insufficient level of water in the steam generator's cooler. The radiation is normal, the ministry said.

Workers exposure during weld tests

Unit N3 is in planned outage. During weld tests in turbine department, 3 workers received doses of 0.65 Sv, 0.1 Sv and 0.03 Sv respectively. Workers' overexposure was caused by defect of testing device, containing radiation emitter. the workers were exposed to radiation during night shift in the period between 11 p.m. of November 9 to 7.00 a.m. of November 10.
Investigation of the event is in progress.

UNIT COLD SHUT DOWN BECAUSE OF THE FLAW IN PIPELINE OF THE PRIMARY MAIN COOLANT PUMP

On December 3, 1995 an increase in radioactivity was detected inside the containment. The visual examination did not reveal the source of the increasing activity. The more careful inspection showed the flaw with diameter 50 mm in the pipeline of autonomous circuit of the primary main coolant pump.
On December 4, 1995 at 12:15 the unit was put into cold shutdown for the failed pipeline maintenance. On completion of the maintenance the Unit was re-connected to the grid.

SHUNT TRANSFORMER INFLAMMATION AT THE HIGH-VOLTAGE (750 KW) LINE SWITCHYARD

On 8 March, 1994 at 0:35 a.m. at the Zaporozhe NPP 750 kW switchyard the "Dneprovskaya" high-voltage line shunt transformer got inflamed.
At 0:50 a.m. in accordance with the plant procedure and by the dispatcher's-on-duty requirement the reactor power was reduced by 500 MWe.
At 4:00 a.m. "the Dneprovskaya" line was back under voltage and at 4:30 a.m. the reactor power was restored up to the previous rate.
The event had no safety or radiation impact neither on the site nor on the region. The event causes are being investigated by a special commission.

RADIOACTIVE CONTAMINATION IN AUXILIARY BUILDING

On January 12, 1994 at 10 a.m. during a routine surveillance walkdown a radiation monitoring operator detected increased background gamma radiation of 0.0015 mSv/h at the entrance of auxiliary building C-117.
Due to leaktightness failure of isolating valves the radioactive medium with specific activity of 1.85-10E6 Bq/l got released from the primary bypass purification system pipeline into the low pressure compressed air system and from the latter into building C-117 in dropwise form.
There was no contamination off-site or personnel exposure.

RADIOACTIVE SPOT AT THE NPP SITE

On June 28, 1993 at 10:20 during a survey of the area by the Radiation Safety Service near by the Special Building no. 1 gate a spot on the ground was found (diameter appr. 300 mm, beta contamination = 500 particles/min., gamma exposure rate did not exceed the rate for the NPP site). Decontamination work was conducted. Dosimetric survey did not find any changes at the NPP site and surroundings. According to the INES the event is classified as below scale.

RADIOACTIVE CONTAMINATION OF THE PART OF NPP SITE

On June 14, 1993 at 9.10 am on the first unit of the Zaporozhye NPP. which was at that time in the process of repairing, radioactive contamination was found on the territory of the transportation corridor of the reactor section. The contamination area is 54 m2.
Dose rate was 0.08 microSv/hour.
The reason of the incident is being investigated by the commission.

HYDROGEN IGNITION ON THE COOLING SYSTEM OF THE TURBOGENERATOR

On May 21, 1993 at 16:28 on the unit 5 of Zaporozhye NPP, which was in planned maintenance, while working at the cooling system of generator no. 5, hydrogen leakage and ignition took place. One of the workers died of scalds at once, the other in grave condition was transported to the hospital.
The ignition was liquidated by switiching off the globe valve on the hydrogen line.

POWER UNIT UNLOADING DUE TO THE THIRD MAIN CIRCULATION PUMP (MCP-3) TURNING-OFF AS A RESULT OF FALSE OPERATION OF MAJOR PROTECTION SYSTEM

On April 19, 19992 at 08:58 a.m. while unit 4 was running at rated power a fire-alarm signal emerged indicating a fire in make-up pumps oil systems compartment. The fire-fighting system pumps were actuated. Inspection of the compartment revealed neither ignition nor fume therefore the pumps were turned off.
