Unshielded radioactive source found in metal scrap

On 29th December 2003 on the customs check-point of Meghri town (on the boarder of Armenia) in the scrap metal shipment which was to be exported from Armenia there was detected a radioactive source. On the source covering the identification mark “C 58191” is weakly viewed. The metallic container of the source was damaged and the major part of the contained material is absent. The dose rate of the source in 5sm distance was 160 mcSv/hour (microSieverts per hour); on the damaged side the beta sensor registered more than 7200 particles/sm2.

Failure of safety system train during routine testing

On July 13, 1998 unit 2 was under 160 MW power. According to the approved programme testing of emergency feed water pumps (EFP) was performed. The test showed EFP-B non-operability. By switching on the EFP-A it switched on, however in 3 minutes it was switched off by the pump protection. Operating personnel did not decrease the reactor power after the testing, as it should be made according to the OL&C. The EFP-A operability was restored in 3 hours 10 min. The EFP-B operability was restored in 6 hours 20 min. A repeated testing was performed without notes.

Reactor discharge due to deterioration of quality of water in the secondary circuit (power reduction)

One June 4, 1998 the plant was under 160 MW electrical power operation. At 10.40 a.m. the shift supervisor received an information concerning high level of content of Na in the samples of blow-down lines of SG-16. In the SG3 and SG6 the content of Na exceeded the level established by L&C (2,3mg/kg and 2,1mg/kg instead of permissible 1,5mg/kg). Operational personnel discharged the reactor power down to 80 MWe. At 14.45 were detected and closed two sources of penetration of Na in the secondary circuit.

Reactor trip due to short circuit on control and protection system of generator

On 4 February, 1997 the plant was under 365 MW electrical power operation with two working turbogenerators. At 6:31 a.m. one of the turbogenerators was disconnected from the grid and tripped by the control and protection system. The reactor power was decreased in order to maintain the parameters of primary and secondary circuits. At 6:34 the reactor was shut down by the control and protection system because of drop of level in the steam generators 1-3.

Reactor trip due to short circuit on the high-voltage line

On October 8, 1996 the plant was under 240 MW electrical power operation with two working turbogenerators. At 7 am one of the turbogenerators was disconnected from the grid and tripped by the control and protection system. Then, in order to maintain the parameters of primary and secondary circuits, the reactor power was decreased by personnel from 240 to 0 MWe. The previous investigation revealed that the cause of generator disconnection was a short circuit at the high-voltage line connecting the NPP with the national grid.
