Overexposure of a radiographer

A radiographer went in a bunker where industrial radiography operations were made. The ionising source was a X-ray machine (225kV - 4mA). The radiographer thought that the irradiation was finished but this was not the case and he was exposed.

According to the biological dosimetry, the radiographer received a whole body dose below 200mSv.

At the moment, the licencee is making a reconstitution of the incident to better determine the dose

Overexposure of a worker

A worker made adjustments on a X-ray machine (65kV - 1.8mA) despite the fact that the device was in test.
The worker received between 5 and 7 Gy on each hands during the intervention. The whole body dose that he received is 1.4 mSv.

Three radioactive level gauges stolen and later found open

On 09.09.2012, the Bulgarian Nuclear Regulatory Agency (BNRA) received a notification for the theft of 3 level gauges from the Polimery JSC. Site, Devnya town. Stolen level gaugescontained Cs-137 sources with activity of 51.5 GBq each (calculated activity for the day of the robbery). The same day, the police found the source containers at a scrap yard, located closely. Containers were broken and the sources were missing.
Later, as a result of police investigation, the thieves were arrested and the missing sources were found, secured and put into a safe storage.

Stolen and lately recovered intact radioactive source

At 21:50, on 03.09.2012, the Bulgarian Nuclear Regulatory Agency (BNRA) received a notification for an industrial radiography camera being stolen. The robbery happened at 15:00 o’clock when the radiography crew had been performing weld checks at a construction site in Sofia (Kosta Lulchev street). The industrial radiography camera is owned by a licensed construction company (license valid till January 2015).
Stolen camera is of the Gammamat R 30 type and is loaded with an Ir-192 radioactive source with a total activity of 380 GBq.

Overexposure to Radiographer’s Assistant

On April 16, 2012, the licensee radiographer’s assistant informed the licensee Radiation Safety Officer that his pocket dosimeter had gone off scale 10 days prior. The licensee sent the radiographer’s permanent badge for processing and the result was a 250 mSv (25 rem) whole body dose reading. The State of Florida was informed of the dose on April 24, and has verified the dose. The State of Florida continues its investigation.

Update 8/15/2012: The radiographer assistant’s film badge was sent for processing.

Radioactive Source Found in Scrap Metal prepared for Export

During the monitoring of the scrap metal prepared for export, LAEC on field inspectors found a high reading on one of the trucks. Upon reloading of the truck, a radioactive source was found to be identified as Cs-137. The source was shielded and safely transported to LAEC for further investigations in the laboratories. LAb analysis confirmed that the source is Cs-137 and that the activity of the source is estimated to be 120 microCi.

Radioactive Source Found in Scrap Metal

During the routine work of LAEC inspectors for monitoring scrap metal prepared for export, a high reading was noticed on one of the trucks. Upon reloading of the truck, a radioactive source was found. The source was safely shielded and brought to LAEC laboratories for further investigation. The result of Lab analysis showed that it is a Sr-90 source of 5 mCi activity.

Worker overexposure

The Ionizing Radiation Calibration Laboratory (IRCL) of the Greek Atomic Energy Commission (GAEC) is equipped with a Co-60 irradiator, used for calibration purposes of radiotherapy dosimeters. In the period 19 - 25 April 2012 the replacement of the Co-60 source (nominal radioactivity of 20 TBq -April 2012) with a “new” Co-60 source (nominal activity of 110 TBq - April 2012) was conducted. The technical part of the source reloading was fully undertaken by the manufacturer of the irradiator (from abroad), who, also, provided the technical personnel (two technicians).

Overexposure to Radiographer

Update 6/4/12: The licensee determined that the radiographer had been distracted by use of her cell phone while performing radiography operations, and her head had been approximately 30 cm (12 inches) below the location of the source for about three minutes, while she set-up the next shot. After exiting the bay, she went to unlock the camera and crank-out the source, but then realized the camera was already unlocked and the source was still cranked out from the previous shot and was positioned inside the collimator.

Discovery of orphan source in a steel mill

On 9 May, a portal monitor alarm was triggered when a truck loaded with scrap from Kenitra (Morocco) came into Siderúrgica Sevillana (a steel manufacturing facility). The truck was isolated within the facility in an area with restricted access to people.
