Overexposure to Radiographer

A lead radiographer approached the end of the guide tube having assumed that the source had already been retracted into the shielded camera. After approximately 90 seconds the assistant radiographer entered the area. The assistant radiographer’s alarming rate meter and survey meter indicated the presence of radiation and the two individuals left the area. The lead radiographer was wearing a whole body dosimetry badge which was sent for emergency processing. The dosimeter was measured by the vendor at 8.36 mSv (836 mrem).

Exposure to workers

On the morning of 14 February 2014, a radiography company was performing non-destructive testing by industrial radiography with a 1,22 GBq Ir-192 source to several pipe joints in a chemical plant. The operations were performed by three operators (A, B and C) in a platform at nearly 12 meters height over the floor. In order to reduce the level of radiation in the area a tungsten collimator was fixed at the tip of guide tube.

Overexposure of radiographers

During radiographic operations performed by two radiographers the guide tube was squashed so the Ir-192 source could not be retracted to the gamma projector. Operations were being performed at 6 m height. The operator tried to correct this situation using a plier to strainghten out the guide tube but unsuccesfully . Then the guide tube was cut and the Ir-192 source was recovered using tongs and shieldings. The workers took personnel dosimeters, alarm dosimeters and detectors but radiation protection officer was absent.

Overexposure to Member of the Public

The Texas Department of State Health Services was informed of an incident involving industrial radiography of a pressure vessel. A site foreman was present in the work area with the radiographer. The radiographer approached the source collimator to change the film and realized the source was still in the exposed position. It is estimated that the site foreman received an unintended dose. The site foreman reported he was in the area for 5-8 minutes and approximately 12-24 inches from the source. A calculated dose for 8 minutes at a distance of 1 foot from the 67 Ci Ir-192 source was 40 rem.

One radioactive source containing Co-60 lost or stolen

In the beaverboard manufacturing plant (in town of Koniecpol, Śląskie region) which is currently under insolvency proceedings (bankruptcy) there has been discovered the loss of one radioactive source containing isotope Co-60. The current activity of lost source is approximately 0,5 GBq. The source was being used in the fill level gauge in the process of beaverboard production. The source was in its shielded container no. PS-5B, weighing approximately 45 kg (please see attached pictures).

Two radioactive sources containing Co-60 lost or stolen

During the modernization in the „Bełchatów” Coal-Fired Power Plant (Rogowiec, Łódzkie province) there has been discovered the loss of two radioactive sources containing isotope Co-60, that were used as the level gauges. The sources were in their shielded containers (dimensions 30 cm x 35 cm x 22 cm). The details of the sources:
- source no BA469 with current activity of 1,08 GBq, container PRJ 500 no 3010 weighing approximately 180 kg;
- source no BA476 with current activity of 2,43 GBq, container PRJ 1000 no 2740 weighing approximately 240 kg.

Found orphan Cs-137 source

On 22.11.2013 radiation portal monitors at a Finnish stainless steel factory indicated that an incoming shipment of scrap metal contained a radiation source. The factory immediately contacted Radiation and Nuclear Safety Authority (STUK). An inspector from STUK went to the site. The source was tentatively identified as Cs-137.

Stolen of a radioactive source

According to the report of Tabriz University Radiation Protection Officer (RPO), a Pu-Be Neutron source was stolen from storage location. National Radiation Protection Department (NRPD) immediately involved in the subject and informed responsible organizations in National and Provincial levels. The source was registered with the activity of 130 GBq (3.5 Ci).

Due to cumulative exposure, a radiographer has received a 51 mSv (5.1 rem) dose for 2012, which exceeds the statutory limit of 50 mSv (5 rem) for a radiation worker. NRC EN49329.
