SEEPAGE FROM A WASTE DRUM

During a routine inspection, about 200 ml of liquid was observed on the building floor adjacent to a plutonium contamined waste drum. Chemical analysis of the liquid indicates a specific activity of 2.2 MBq/ml Beta. Two people received minor contamination of their hands while taking a sample of the liquid. They were successfully decontaminated. The quantity spilled is estimated to be 450 MBq of plutonium, radiologcally equivalent to 4500 GBq of ruthenium 106.

PLUTONIUM RECOVERY PLANT, B204

An abnormal discharge of plutonium amounting to 600 MBq over 2 days took place via the ventilation system and discharge stack from a redundant plutonium recovery plant. The current best estimate of dose to the potentially most exposed member of the public is about 1 microSv.
An investigation is underway to determine the cause of the release.

SIGNIFICANT SPILLAGE OF PLUTONIUM LADEN SOLVENT

The final stage of reprocessing of spent nuclear fuel involves the separation of plutonium in a solvent phase (tributyl/phosphate/odourless kerosene) from the uranium/plutonium/fission product mixture. The solvent containing plutonium as nitrate at about 10E+6 Bq/ml is sampled in a number of lightly shielded cabinets before the solvent is stripped to leave plutonium nitrate in the aqueous phase.

PLUTONIUM NITRATE LEAK IN CELL

Plutonium nitrate leaked from a corroded pipe to the containment cell and accumulated as a crystalline solid mass. There was no release to the Operations Areas nor to the Environment. The spillage was removed, the plant repaired and restarted.

TEMPERATURE TRANSIENT DURING START-UP

During power raising the channel gas outlet temperature (CGO) on one channel exceeded the Operating Rule limit for a short period. At the outset of this period the need for corrective action was recognized and bulk rods were inserted at an increased but insufficient rate to terminate the temperature transient. The rate of insertion did, however, ensure that the temperature rise was controlled.
BASIS FOR RATING: 1) Real initiator: controlled power and temperature increase during start-up within the auto-reset rate of trip protection - frequency - expected.

ISOLATION OF FUEL OIL TO ESSENTIAL GAS TURBINES

During test of a gas turbine it was discovered that fuel oil supplies to all gas turbines had been inadvertently isolated 18 hours earlier. Fuel oil supplies were restored within one hour.
BASIS FOR RATING: Safety function availability: adequate as in the event of loss of grid, several hours would have been available to restore supplies.
Postulated initiator: loss of grid.
