Fuel fail and emission from research reactor

Fuel rods were unloaded October 17 2016, and placed in an open container in the handling compartment. The reactor was then in shutdown. On October 20th increased levels of noble gasses were detected in the filters from the handling compartment. The fuel in the handling compartment was inspected and several faults on fuel rods were detected.
These were moved to a storage container on October 24, however the container was discovered to not be leak tight. The levels of I-131 and I-132 in the Reactor hall were still increasing.

Electronic Personal Dosimeter went into Alarm during X-ray Radiography Operations

Radiographers were working in a cell in a redundant reprocessing facility, carrying out digital imaging of legacy crates using X-rays in order to identify their contents.
This cell is an area of known high background radiation containing ten legacy crates.
A previous health physics survey of the crates had identified radiation levels in nine of the crates between 0.5 mSv/h and 1.8 mSv/h gamma, with the other crate up to 20mSv/h gamma.

Transport of package containing a radioactive source from Kenya to South Africa

A parcel containing an industrial radiography sealed source was transported from Kenya to South Africa. The package was labelled as empty (UN2908) and was handled as a general luggage during transportation in a South African Airways passenger flight. On arrival at the final destination the dose rate on the outside surface of the package was measured by the consignee to be 6mSv per hour. The measured dose rate on the surface of the package could potentially result in the low to moderate health risk to people who were in contact with the package.

Inadvertent damage of a Se-75 source

On December the 12th, 2016, a Se-75 source was inadvertently damaged during re-pair work on faulty radiography equipment. The work was performed in Düsseldorf region in North Rhine-Westphalia by a specialized company within a controlled area. The source contained about 1.3 Terabecquerel Se-75. The radioactive substance was released during the repair work and contaminated persons and zones within and out-side of the controlled area. Besides the two workers performing the repair work, other persons were also contaminated.

Cyclotron operated with an open bunker door

On the 12th of October 2016 the Swedish Radiation Safety Authority was informed about an event that occurred on the 7th of October where a cyclotron was operated with an open bunker door. The licensee is authorised to produce short lived isotopes used for medical

Overexposure of a member of the public

An industrial radiograph test was performed in Sakarya province, Turkey on October 17,2016 after which a serious radiological incident occured.
Test was performed in a normal manner but after the test, the radiological protection officer hurried up and skipped the control that the source was secured. The source, Ir-192 with a 15 Ci activity (category 3 source according to INES manual) at the date of the accident, dropped to the worksite as unsealed. The source was found by a 16 years old visitor (E.B) approximately at 03:40 p.m. (local time) next day. E. B. placed the source to his trouser’s back pocket and travelled to home for two and a half hours. At the house, household members and their guests declared that they touched or saw the source until approximately 09:00 a.m. on October 20, 2016.
The source was reported as lost to Turkish Atomic Energy Authority Disaster and Emergency Management Center by its owner on October 20, 2016.

Excessive Deposits of Material Containing Uranium In Scrubber

At a facility authorized to use low-enriched uranium to fabricate commercial nuclear fuel assemblies, excessive deposits of uranium-bearing material were found in the main scrubber and associated ventilation ductwork. The function of the scrubber is to remove gases and particulates from various process exhaust streams. During the most recent planned annual wet scrubber system cleanout, personnel noticed an abnormal amount of material buildup in the inlet transition region and associated ductwork (i.e. elbow). Over the course of the 2-day maintenance evolution approximately 197 kilograms of material were removed from these two sections. These sections are not a favorable geometry from a criticality perspective. Since the facility personnel were under the assumption that this material had a low uranium concentration, operators attempted to break up and wash away the material to facilitate its removal. The facility personnel did not sample the material to confirm the uranium concentration before conducting these activities. After the material was removed, grab samples of the material were taken and analyzed for uranium concentration. The grab sample results indicated that the uranium concentrations ranged from 34wt% – 55wt%, which corresponded to approximately 87 kilograms of uranium. As such, the criticality safety evaluation mass limit of 29 kilograms was exceeded by a factor of 3.
