Theft of two low activity radioactive sources in Palaiseau

The French Nuclear Safety Authority (ASN) was notified on June 14th about the theft of two portable devices for detecting lead within paintwork containing each of them a low activity Cobalt 57 source.
These devices were stolen in Palaiseau (South of Paris) during a transportation carried out by the FEDEX Company, just before their delivery to the PROTEC Company, their official provider in France. The lorry carrying them was entirely cleaned out of its load following to an armed robbery.

Small leakage from Residual Heat Removal System.

Temelin Unit 2 (VVER 1000 Reactor type) was in shutdown from June 2, 2004 for replacement of main output transformer (2AT3). The reactor heat removal was established by the residual heat removal system (2TQ32). Spontaneous step increase in flow indication (2TQ32F001) was detected in the morning hours on June 6, 2004, complemented by a slight decrease in pressure indication on respective residual heat removal pump suction. The immediate inspection on the pump (2TQ32D01) identified a pipe leakage on the impulse pipeline of the referenced flow measurement.

Contamination of Demineralized Water System & Uncontrolled Minor Radioactivity Release

Event Description

1. Circumstances at the initial stage of the Event
On December 22, 2003, at 23:32, just prior to the plant start-up, an "Alert" alarm of the radiation monitor (RM, RE-054) monitoring the radiation level of intake air of Technical Support Center HVAC actuated. Plant start-up was proceeding in parallel with checking of the cause of RM alarm actuation. Full power was achieved on December 24, 2003.

Fire on electric cables in Cattenom-2 NPP

On May 16th, 2004, a fire has started at about 12:15 in an equipment room containing electrical cables on the unit 2 of Cattenom NPP located in the French Moselle department.
EDF has triggered the on-site emergency plan and has shut down the unit 2. Firemen have intervened and have considered the fire extinguished at 14:25.
There is no people injured nor consequence to the environment.
As a precautionary measure, the Nuclear Safety Authority (ASN) has activated its emergency centre and its representatives have gone to the Moselle Prefecture and to the NPP site.

Loss of a type A package between Saclay (France) and Nuremberg (Germany)

On 23 February 2004 the Schering company (CIS bio international) notified the French Nuclear Safety Authority (ASN) that a type A package containing radioactive material for radiopharmaceutical use, shipped on the 20 February, has been lost during its transport between Saclay (France) and the consignee, an hospital located in Nuremberg (Germany).
This type A package consigned by CIS bio international (France) contains a limited quantity of Rhenium 186 (At the time of the shipment the total activity was 374 MBq).

Process vessel with natural uranium compound found in stainless steel scrap

On 16 December 2003 JEWO-metaal B.V. alarmed the VROM-Inspectorate that they had a radiation alarm of their portal detector. A container with stainless steel scrap from Jordan had a radiation level of 0.3 microSievert per hour at contact. The enhanced radiation level was caused by a process vessel of about 40 liters inside the container. The radiation level at the outside of the vessel was 2.2 microSievert per hour. The process vessel contained a filter with about 1 kilogram of natural uranium as remnants of a brown and yellow filtercake.

Incidents of Radioactivity in Consumer Products

Three incidents of radioactivity in consumer products were brought to the attention of the Radiation Protection authorities in Canada. The products involved were stainless-steel thermoses, canisters and watch bands. The components found to be contaminated with cobalt-60 were the clips holding the handles to the thermos body, the ring holding the canister lid in place and the spindles (pins) in metal watch bracelets. The contaminated watches and thermoses originated in the People's Republic of China. The origin of the canister is unknown.

Steam Generator Tube Failure

Steam generator tube failure event occurred at Ulchin unit 4 on April 5, 2002. Ulchin unit 4 was in shutdown mode to initiate cooling down at 01:20 on April 5, 2002 for the 3rd refueling outage. At 18:33 (all times are KST) the operators observed notable decreases in the pressurizer level and pressure at the beginning stage of cooling down. In reponse, the letdown flow was isolated and a third charging pump started to compensate the loss of primary coolant inventory at 18:55. Even though a third charging pump was activated, it couldn't make up the loss.

THEFT OF NUCLEAR GAUGE

In April 9th, 2003, a nuclear gauge was stolen from a pick up truck while parked in front of owner offices. A dennounce was made by the owner to the National Police. The nuclear gauge was inside its normal transport package. The radioactive sources inside the gauge are 296 MBq of Cs-137 and 1,48 GBq of Am-Be. The radiation level around the nuclear gauge are nearly 50 uSv/h at most.

The nuclear gauge was unauthorized by the regulatory body.
