A sealed radioactive Cs-137 source contaminated the premises of waste management company

Event Abstract:
A sealed radioactive Cs-137 source contaminated the premises of waste management company and also the neighboring rooms used by Finnish Radiation and Nuclear Safety Authority (STUK). The event was found out on 7 March 2016.

The source, activity presently 360 MBq, had been used in a factory in Finland for level gauging and was delivered to waste management company as radioactive waste. When the gauge was received by the company, one of the staff tried to open the source shield in order to remove the actual cesium source for disposal. He did not have information or other indication that the source might not be intact. During the handling of the source contamination was released into the room where gauge was handled and also to adjacent rooms and air exhaust ventilation ducts.

Leak from primary coolant system at Kakrapar Atomic Station-1

On March 11, 2016, Kakrapar-1 was operating at 220 MWe. At about 0852 hrs, primary containment (pump room) pressure high alarm appeared in control room. Both fast acting reactor shutdown systems and liquid poison injection system actuated automatically and brought the reactor to long term sub-critical state. Emergency Core Cooling System (ECCS) actuated as per design and provided cooling to the core.

As per the established procedure, plant emergency was declared at 0910 hrs.

Shutdown cooling loop was taken in service and primary coolant temperature was brought to 45 °C.

Stolen pick-up truck with Ir-192 cat II source

In July 3rd. 2014, at the Municipality of Atizapán de Zaragoza, Mexico State, Mexico, a pick-up truck was stolen, this vehicle was transporting an Industrial Radiography Ir-192 radioactive source, with activity of 1.23 TBq, category II.

On July the 4th, 2014, the vehicle was found at the municipality of Tlalnepantla, Mexico State, Mexico.

Missing Dangerous Source at Salamanca, Guanajuato

In February 3rd. 2015, the National Commission on Nuclear Safety and Safeguards (this is Mexico’s nuclear regulatory authority) was informed by a licensee that three vehicles of its property had been stolen, each one was transporting an Ir-92 source. Therefore, three sources were missing. On the same day, after carrying out an investigation the sources were located and secured by the regulatory authority staff. All the sources are intact and safe. .

Missing Dangerous Source at Cárdenas, Tabasco, México

On April 14th 2015, the National Commission on Nuclear Safety and Safeguards received a report about the theft of a vehicle transporting a radioactive source. This had occurred in the municipality of Cardenas, in the State of Tabasco, Mexico. The stolen source was Ir-92 with an activity of 32 Ci. The source category is 2.

On April 22nd 2015, due to the information given by an anonymous phone call to the State Police, the container with the radioactive source was located and recovered.

Orphan source found

On 20th May 2015 the RSD's inspectors were informed by an authorized technical service on increased radiation in a truck loaded with metal scrap (aluminium, copper and messing), with dose rate of 368 μSv/h measured on the surface of the truck.
The truck was loaded in metal scrap yard in Skopje, Macedonia and it was intended for export from Macedonia to a facility in Sofia, Bulgaria.

Eu-152 lightning rods found in metal scrap

On 15th May 2015 the Radiation Safety Directorate's inspectors were informed by an authorized technical service on increased radiation in a truck loaded with metal scrap (aluminium, copper and messing), with dose rate of 84 μSv/h measured on the surface of the truck.
The truck was loaded in metal scrap yard in Veles, which collects metal scrap from allover of Macedonia and it was intended for export from Macedonia to a facility in Ljubljana, Slovenia.

Found category 4 source, shutter of the source container partly open

On 14th March 2015 Italian border control detected increased radiation levels at an Austrian railcar at the Austrian border station. The railcar transported scrap metal. Austrian radiation experts found a Cs-137 category 4 source with 2.5 GBq activity. The shutter of the source container was partly open. The radiation source was secured and transported to the Central Austrian Waste Management Facility at Seibersdorf.

The owner of the Cs-137 source was identified by the responsible Austrian authority.

Stolen radioactive sources

On the 6th of March 2015 total of 22 containers with Co-60 category 5 (activity between 40 MBq and 200 MBq on 2015-03-06) radioactive sources were reported stolen from a storehouse in Poznan. On the 10th of April the licensee informed that 7 of the containers previously reported stolen have remained intact inside the storehouse. Aggregated activity of the stolen sources equals about 1,7 GBq.
