Radiographer Overexposure

The Oklahoma Department of Environmental Quality was informed of an overexposure of a radiographer assistant employed by Western X-ray Services. The dose involved is 51 mSv (5.1 rem). On seven occasions during 2012, the dosimetry vendor had contacted Western X-Ray Services to inform them of the high dosimetry dose readings for the radiographer assistant. The supervising radiographer whom the radiographer assistant had worked with stated that there were no unusual incidents or equipment problems with their QSA Global model 660 exposure device and the Ir-192 sources. It was concluded that Western X-Ray Services had not monitored the assistant’s exposure closely enough. Western X-Ray Services requested termination of their license. NRC130131
