Radiographer Overexposure

On February 22, 2006, the State of California notified the Nuclear Regulatory Commission (NRC) that a radiographer’s assistant may have received a calculated shallow dose of 1.28-3.58 Gy (128-358 rad) to either hand, a dose in excess of the NRC’s annual occupational extremity limit of 0.5 Sv (50 rem).

According to the licensee, during the conduct of radiographic operations on February 22, 2006, a radiographer’s assistant approached the device containing a 2.3 TBq (63 curie) iridium-192 sealed source to disconnect the source guide tube without a survey instrument. Once the source guide tube was disconnected, the radiographer noticed the source pigtail protruding from the end.

The licensee sent the personal dosimeters worn by the individuals for analysis. The radiographer’s assistant’s direct reading personal dosimeter was off-scale and the radiographer’s had a reading of 120 microSv (12 mrem).

The State of California is investigating the event.

The Provisional rating for this event is Level 2.
