REACOR SCRAM AND TURBINE TRIP BY RCP BUS LOW VOLTAGE

The unit was at 240 MWe operation before the event occurred. At 4:16 several alarm signals occurred as mentioned below:
1) the signal of 6 kV bus I and Bus II voltage circuit breaker, 2) the signal of 380 V bus voltage circuit breaker, 3) the signal of the AC voltage of kilowatt-hour meter of 220 kV bus I disappearance.
In the meantime, the voltage of generator and 6 kV bus declined which made the RCP bus voltage too low and resulted in reactor scram and turbine trip.
The event was due to badly worn exciter carbon brushes which resulted in generator excitation shortage and generator low voltage which caused 6 kV bus I voltage to decrease. The 6 kV bus I low voltage signal initated the reactor protection system and resulted in the reactor scram and turbine trip.
The event was rated with level 0 (Section 4, Table IA-1R).
