REACTOR SHUTDOWN CAUSED BY AN EMERGENCY PROTECTION SYSTEM ACTUATION FOLLOWED BY NON-COMPLIANCE WITH THE SAFETY LIMITS

On September 21, 1992 the unit was restarted after planned maintenance and was gaining power. At the power level of 450 MWe at 7:52 a.m. (Kiev time) the reactor was shut down after the emergency protective system on neutron flux level actuation. The actuating signal had been formed as a result of a defect in the reactor neutron flux control system elements. During the transient process, because of the failure of the fast-acting isolating valves of the steampipe (protective systems affecting valves were not in preparedness state), there was a pressure drop in the steamgenerators and of the level in the pressurizer lower than the prescribed limits for safe operation.
